How does an AI voice agent work?
An AI voice agent works by running three systems in a fast loop: speech recognition turns the caller's words into text, a large language model decides what to say and which action to take, and text-to-speech turns that reply back into a voice. Each stage streams into the next, so the reply starts within a second or two. Outpero's agents run this loop at 0.8 to 1.3 seconds on average end to end.
The listen, think, speak loop
Almost every production voice agent today is a pipeline of models connected to the phone network. The call delivers audio; the agent listens, thinks and speaks, then listens again. That cycle repeats for every turn of the conversation, dozens of times on a longer call.
- 01Speech recognition (ASR). The caller's audio is transcribed into text as they speak, word by word, rather than after they finish.
- 02Turn detection. Voice activity detection and endpointing decide when the caller has actually finished, as opposed to pausing to think.
- 03Reasoning (LLM). A large language model reads the transcript so far, the business's instructions and any facts it has been given, then writes the next reply or decides to use a tool such as a calendar.
- 04Speech synthesis (TTS). The reply is converted into audio and streamed down the phone line, starting before the full sentence has been generated.
Why streaming matters
If each stage waited for the previous one to finish completely, a reply would take several seconds and the caller would assume the line had dropped. Streaming lets the stages overlap: the language model begins writing as soon as the transcript is final, and the voice begins speaking its first few words while the rest of the sentence is still being produced.
This is why latency, not raw model intelligence, is usually the hardest engineering problem in voice AI. A brilliant answer that arrives three seconds late feels worse on a phone call than a good answer that arrives in one.
What happens around the conversation
The loop is only the middle of the call. Before it, a dialler places the call over the phone network and checks whether a person or a voicemail answered. After it, the full transcript is summarised, scored for sentiment and mined for the specific fields the business asked for, such as budget, preferred date or locality.
How Outpero does it
Outpero's voice engine, Swara, runs this pipeline in Hindi, Telugu, Tamil, Marathi, Kannada, Malayalam and Indian English, at 0.8 to 1.3 seconds on average from the moment the caller stops to the moment the agent speaks. During the call it can book a calendar slot, read or write a spreadsheet, call a custom API or send a WhatsApp message, each switched on per agent. After the call, the recording, transcript, summary, sentiment and captured fields land in the dashboard and on your webhook.

Follow-up questions
Does an AI voice agent follow a script like a human telecaller?
+
It follows instructions rather than a fixed script. The business describes the goal, the tone, the facts the agent may use and the questions it must ask, and the language model writes natural sentences within those limits. That is why it can handle an unexpected question without breaking flow, and also why clear instructions matter more than clever wording.
